Arkansas

The Arkansas Rural Health Transformation Program opened applications for the RISE AR (Recruitment, Innovation, Skills, and Education for Arkansas) initiative, which supports rural healthcare workforce recruitment, training and retention efforts. Applications are due by July 24 at 12:00 pm CT and applicants are encouraged to review the NOFO and program materials for eligibility and submission requirements.

Minnesota

The Minnesota Rural Health Transformation Program released a new funding opportunity to support the development of rural clinical fellowship programs for advanced practice providers, behavioral health professionals and oral health professionals. The Rural Clinical Rotations Expansion Grant Program aims to strengthen the rural healthcare workforce through community-based, post-graduate training, mentorship and interdisciplinary learning experiences that prepare clinicians to practice in rural communities. Applications are due July 31, and an informational webinar will be held on July 1. Additional funding opportunities remain available for telehealth access points, tele-buprenorphine services, rural residency training programs and expanded rural clinical rotations. Mississippi

Pharmacy stakeholders across a variety of associations as well as health systems, independent and chain pharmacies, the Department of Health and the Division of Medicaid have come together to represent a united effort for pharmacy in the state's rural health transformation program, which received $205 million in funds this year to distribute across the state that builds and cultivates local healthcare infrastructure and successful patient outcomes.  

An area of interest to the governor's office is a state-wide healthcare digital backbone. Pharmacy can and should be included in this. As a first step, the Mississippi Pharmacists Association in partnership with the University of Mississippi School of Pharmacy is collecting a baseline assessment of digital readiness in pharmacies that can show improvement over the course of a one-year grant, which will set pharmacies across the state up for success in the state-wide Health Information Exchange (HIE). 

Please complete and submit one response of this survey per organization or pharmacy NPI; if multiple team members contribute, designate a single coordinator to compile and enter the final responses. The application portal officially opened for the Promoting Access, Coordination and Transformation (PACT) initiative, a key component of the state’s Rural Health Transformation Program. The initiative is designed to strengthen and modernize rural healthcare by expanding access to care, improving coordination among providers and supporting integrated networks, workforce development and technology-enabled infrastructure. Applications are due by 12:00 pm CT on July 10. The survey was intended to gauge early interest and does not commit organizations to participate.

Alaska

The Department of Health Implementation Application Walkthrough Webinar (June 1, 2026) is now posted. Of the nearly 1,800 Letters of Interest received from 864 unique organizations, 403 were advanced to this full implementation pathway funding, while 28 were approved for planning pathway funding. 

Also in Alaska, the Department of Health is also hosting regional planning meetings across Alaska, with upcoming sessions scheduled for Mat-Su on June 4 and the Greater Anchorage Area on September 22. Interested stakeholders can submit their Regional Meeting Interest Form or provide input through the Community Input Form 

Finally in Alaska, the Department of Health anticipates opening a new Letter of Interest (LOI) submission period for Year 2 funding in early fall 2026, along with state-directed targeted innovation opportunities focused on priority areas such as equipment upgrades, training and value-based payment models.

The state's Rural Health Transformation Program released a Notice of Funding Opportunity (NOFO) for PACT (Promoting Access, Coordination, and Transformation) initiative. The program is designed to strengthen and modernize rural healthcare delivery through expanded access to care, improved care coordination, workforce development, clinically integrated networks and technology-enabled infrastructure. 

The PACT initiative will support projects focused on: 

  • Expanding access to primary, specialty, preventive, behavioral health and emergency services in rural communities;  
  • Improving coordination among hospitals, clinics, EMS providers, pharmacies and community-based organizations;  
  • Strengthening trauma readiness and emergency response capabilities;  
  • Supporting clinically integrated networks (CINs) and shared data infrastructure;  
  • Enhancing workforce training and optimization; and  
  • Advancing telehealth, technology adoption and strategic facility investments.  

The application period is open June 8 through July 10, with approximately $93.6 million available through the initiative. 

Notably, pharmacies are specifically identified as eligible applicants, along with EMS providers, licensed healthcare practitioners, behavioral health providers, CINs and nonprofit or public entities serving rural Arkansas.

Colorado

Colorado scheduled multiple webinars to support RHTP grant applicants, including a Partnerships for Grantees session on June 10, from 2:00-3:00 pm, focused on creating meaningful partnerships with healthcare organizations, community groups and vendors. A comprehensive How to Fill Out the RFA webinar is scheduled for June 23, from 9:00-10:30 am, offering 90 minutes of tips and recommendations for completing grant applications. 

The state also provides ongoing support through regular office hours (see below), along with an RHTP Advisory Committee meeting on June 22. Colorado published an Available Vendor Partnership List to help providers identify potential collaborators, and vendors can express interest through a dedicated vendor interest form.

Iowa's Healthy Hometowns program continues to expand its Rural Health Transformation Program through new funding opportunities, including the Combat Cancer Health Hub Program and the Maternal Health Hub and Spoke Networks of Care RFI. The University of Iowa Health Care and Iowa Primary Care Association have been selected by the Department of Health and Human Services as the statewide Health Hub Technical Assistance Provider for the state's Rural Health Transformation Program and Healthy Hometowns initiative. They will provide training, resources and guidance to organizations interested in developing Health Hubs. 

University of Iowa Health Care will host a kickoff webinar on June 23 from 10:00 am to 12:00 noon CST to provide an overview of the program, technical assistance (TA) goals, available support services and 2026 educational opportunities. Two major RFP deadlines are approaching for the state's Rural Health Transformation Program infrastructure. The Administrative Services Organization (ASO) RFP application is due June 5 at 5:00 pm MST, with amendment documentation and FAQ guidance available. The Center for Rural Health Sustainability & Innovation (CRHSI) application is available, with a June 12 deadline and is supported by RFP documentationamendment details, and FAQ resources. 

Additional funding opportunities include the Healthy Horizons RFA application due July 2, which focuses on connecting rural communities to specialty, maternal, behavioral health and chronic disease care services. Three new funding streams are scheduled for release: the Rural Health Innovation Fund on June 8, Rooted in New Mexico in mid-to-late June and the Rural Health Data Hub in late June. Wyoming

Gov. Mark Gordon (R) and US Senator John Barrasso (R) announced CMS granted its final approval of the state’s Year 1 application for $205 million in federal funding through the Rural Health Transformation Program (RHTP). The state submitted an updated proposal to the federal government in early May and CMS approved the updated proposal.
